Help, need a quick fix code!!

Hi Guys

I'm looking for a quick fix code.

I need a form with a text box input and a submit button.
And the action I need is:

1. Enter 'Hello' (or whatever) into the text box
2. Click the submit button
3. It takes you to the link www.google.co.uk/Hello
